In our Musculoskeletal Rehabilitation class, we were required to do SOAP notes for our clients, two each week, to help us get more familiar with the documentation process and learning what is necessary and what is not. With this, it shows at the end of the class I was able to get all the necessary information to complete a SOAP note. The documentation is accurate, concise, timely, legible, and grammatically and technically correct. The documentation is different for every clinic, so it is important to basic information because you will constantly be working with different documentation systems.
